- New students applying to the Faculty of Health Sciences (except Non-Regular Nursing and transfer students) must be declared healthy based on a medical test, drug-free, and not totally color-blind. They must upload the laboratory test results after re-registration. Accepted test results are only those issued by a public health center (puskesmas), a regional public hospital, or BioMedilab clinic.

- The student submits to BAAK the academic evaluation form approved by the Head of Study Program, which includes:
– Study program
– Effective year level
– List of credited/accepted credits (SKS) from the previous institution
– Remaining credits (SKS) to be taken at HorizonU, broken down by semester
– Courses to be taken each semester - BAAK will input the credit conversion results into SIS and create the student’s SIS account.

The student checks their email for SIS login credentials (username, password, and login link). Go to apply.horizon.ac.id using the username and password sent via email, then change the password.

- Complete the personal data required in the application portal.
- Make the enrollment fee payment via bank transfer.
- After payment verification, you will receive a welcome letter containing your Student ID Number (NIM) and SIS login credentials via email.
- Log in to SIS using your username and password.

- Log in to SIS using your username and password.
- Complete your profile under the “Student Details” menu, then go to “Student Management” and finally “Student Profile.”
- Upload required documents to SIS (ID card, family card, diploma, transcript, birth certificate, photo, medical certificate, NISN).
- Go to the Student Registration menu, choose your class and schedule, then submit.
- Once approved, download your KRS.
